What to Expect At Your First Visit
Your first visit to Miracle Dental is an important one as it introduces you to our practice and our friendly staff. During the first visit, we will collect background information like medical history as well as take the time to get to know Dr. Pham. Below you will find additional information to view before your first visit to Miracle Dental. Also, to help ensure your first visit goes as smoothly as possible, we invite you to take a few minutes to explore our website to educate yourself about some of the dentistry and smile solutions we offer.
Insurance & Financing
If you have any questions about your dental insurance coverage, please contact our office and let our staff assist you. Our office also accepts CareCredit. Printable Patient Forms
If you are a new patient to our office, please fill all 5 of these forms out and bring them with you upon arrival to your first appointment. Each individual needs a packet of 5 forms for him or herself.
These forms are fillable from your computer except for a few areas that require signatures. If you don’t have a printer at home, please fill them out, save them, and email them back to our office at miracledentalnv@yahoo.com. We will be glad to print them out and you can sign them once you arrive in our office.
